Hackensack Meridian Health (HMH), in partnership with NJ FamilyCare, NJ Health Care Quality Institute and the Visiting Nurse Association of Central Jersey, has entered into a collaborative agreement with Centers for Medicare and Medicaid Services (CMS) to establish the NJ Integrated Care for Kids (InCK) model. NJ InCK is a multi-sector collaborative child-centered local service delivery model aimed at reducing out-of-home placement as well as health care expenditures for children covered by NJ FamilyCare. It operates through prevention, early identification, and treatment of priority health concerns like behavioral health challenges, physical health needs, and social challenges. For those with the highest need, cutting-edge information systems and advanced case management programs will ensure the integration of clinical care and a wide variety of social services. Beginning January 1, 2022, pediatric providers who implement the NJ InCK Needs Assessment for enhanced EPSDT screening and care planning may be eligible for expanded payments for Ocean and Monmouth County families enrolled in NJ FamilyCare.

 

TIME SENSITIVE INFORMATION:

The open submission period for the NJ-InCK Needs Assessment Tool RFP has officially closed as of February 4th, 2021.  The RFP was developed to solicit responses for a technology to capture screening information of a targeted pediatric population in Monmouth and Ocean counties in New Jersey. The ideal  offering can be delivered to patients and families as an automated self-assessment tool with multiple assessments based on the patient. Any solution introduced into the NJ-InCK environment must conform to NJ-InCK security policies, performance standards, and code review.
