Please follow the detailed instructions on the screening log. Important information to remember (pdf):

  1. This log is to be completed by EACH participating provider
  2. Include physician name and practice name
  3. Remember to fill out the weekly start and end dates
  4. Include the total number of well visits for the week – for the provider, not the entire practice.
